AS A PATIENT, YOU ARE RESPONSIBLE FOR –

GRIEVANCE PROCEDURES –
AMENDMENT TO CHARGES FOR MEDICAL RECORDS –

GRIEVANCE PROCEDURE:

You have the right, without recrimination, to submit a grievance either verbally or in writing regarding your care, to have those complaints reviewed and, when possible, resolved.

Complaints may be given to any staff member who shall immediately address them and forward them to the clinical director. You may also contact the clinical director: Sharon Tkaczuk, c/o Southwestern Pennsylvania Eye Surgery Center, 750 East Beau St., Washington, PA 15301
